Course Summary

The Centers for Disease Control and Prevention reports that one in four adults in Colorado has some type of disability. The process of transition from child to adult for individuals with disabilities is complex. This course is designed to provide you with knowledge, tools, and insights to provide youth-to-adulthood transition support to people with disabilities. You will gain information and skills to navigate the complexities of long-term services and supports available to eligible youth with disabilities transitioning to adulthood.


Learning Objectives

Upon successful completion of this course, you will be able to:

  1. Identify major legislative acts in the United States that address the rights and needs of youth with disabilities during the transition to adulthood.
  2. Understand the fundamental differences between educational transitions, which focus on academic and vocational goals, and healthcare transitions, which address medical, therapeutic, and wellness needs.
  3. Determine available resources, programs, and services aimed at supporting the transition of youth with disabilities to adulthood, including government-funded initiatives and community-based organizations.


Learning Audience

The learners for this training are behavioral health service providers, social workers, counselors, peer support specialists, and other professionals who support individuals with substance use and behavioral health conditions.
