Participants will be able to strengthen communication, coordination, and shared decision-making practices that bridge service gaps for rural patients, ensuring continuity of care across settings and practitioners.
Learning Objectives
Upon successful completion of this course, you will be able to:
- Define what care coordination and the continuum of care mean within rural and frontier contexts
- Identify barriers to effective coordination in remote or resource-limited areas
- Apply communication and documentation practices that support integrated care
- Recognize how to use regional networks, technology, and telehealth tools to maintain continuity of care
- Implement practical strategies through shared responsibility to respond effectively when a patient’s needs extend beyond one’s scope, setting, or available resources
Learning Audience
The learners for this training are behavioral health service providers, social workers, counselors, peer support specialists, and other professionals who support individuals with substance use and behavioral health conditions.
Continuing Education Credits
This course is eligible for Nursing Continuing Professional Development (NCPD) credit upon completion, which are awarded as follows:
